During his playing days, O’Neill played for eight different football clubs: Distillery (1971), Nottingham Forest (1971-81), Norwich City (1981), Manchester City (1981-82), Norwich City (1982-83), Notts County (1983-84), Chesterfield (1984) and Fulham (1985).

For his career, he played in 435 matches, scoring 68 goals while playing as a midfielder. Internationally, he played for Northern Ireland from 1971-84, scoring eight goals in 64 appearances.
